Smoking

The Board of Directors and staff of North Central Health Care are committed to providing and promoting a healthy environment for you and your visitors. As a part of this commitment, we have chosen to pursue the goal of a tobacco-free environment within the facility. By becoming tobacco-free, we believe we will assume a role of health care leadership.

Visitor and employee smoking occur only in designated outdoor areas.

In the Health Care Center building, smoking continues for residents within the designated areas.

We appreciate and thank you for your cooperation and attention to our smoking policy and tobacco-free goal.

Alcoholic Beverages

Residents may have alcoholic beverages with the approval of their attending physician. Any alcohol brought to the facility must be stored at the nursing station. Please keep in mind that other residents may have health conditions or medications, which disallow alcohol consumption, therefore, sharing alcohol with peers is not appropriate in the nursing home.

Access to Records

You may ask to review your records at any time. Your nurse, physician, and/or social worker will arrange this opportunity for you and be available to answer any questions you may have. Your medical record is confidential information and is only seen by those persons involved in your care and treatment and to officials, surveyors, or auditors as authorized by Wisconsin law. Family and others may review records with written informed consents, reasonable notice, and during reasonable business hours. Copies of records are available for a reasonable charge.

In the same manner, we will not give information about you to any caller without your consent.
