| Concerns and Grievances Our facility is committed to providing efficient, quality care, which is always respectful of the individual and dedicated to customer service. We have established an abuse protocol, which identifies our procedures to safeguard our residents from abuse, neglect, or misappropriation of property. We are always open to suggestions for improving services. You or your family have the right to comment, to express concerns, or to voice a grievance (complaint) about your treatment, care, management of funds, or violation of rights, without discrimination or reprisal. You may register a comment, concern, or grievance at any time with any staff member or outside agency of your choice. Written complaints can be submitted on a grievance form available from staff. Staff will assist you in writing the grievance if you request. The Client Rights Specialist will investigate your grievance and a prompt, written response will be sent to you. You may contact the Long Term Care Ombudsman at the Board of Aging and Long Term Care, or you may contact one of the advocates listed below if you choose to raise a concern or file a complaint.
