Community Treatment North Central Health Care offers a variety of supportive services for those in our Central Wisconsin communities suffering from persistent to severe mental illnesses, or struggling with substance abuse. Adaptability and innovation have made our programs very successful in helping people to function at their maximum capabilities in the community.

In Community Treatment, services are deployed in community-based settings — such as an individual’s home or workplace — and are geared toward achieving successful outcomes customized to each individual’s lifestyle.

Who Can Enroll in Community Treatment?

North Central Health Care’s Community Treatment programs serve individuals coping with substance abuse, mental health issues or co-occurring disorders. We provide support within the community by providing individualized recovery-oriented services that are both flexible and empowering.

Determining Care, Treatment and Support

We help individuals gain access to and coordinate a full array of services including medical, social, educational and vocational services. Individuals are matched with a case manager who assesses needs, helps develop a plan around individual goals, monitors progress and provides continual service coordination.
