Below are the latest reported positive case details reported at Pine Crest with residents and employees. This page will be updated daily with new case details. Responsible parties for residents will receive a once weekly phone update that will report on the status of cases and any information that you may need regarding operations and visitation. NCHC is in close contact with the Lincoln County Health Department and is following recommendations by the CDC and local health officials. We will continue with regular routine testing of residents and staff as required according to current policy.

Since our last phone notification on April 10 there have been no new resident or employee Covid-19 cases reported and there are currently no active resident cases at Pine Crest. Since our last phone notification on March 26 there have been no new resident or employee Covid-19 cases reported and there are currently no active resident cases at Pine Crest. Our team continues close contact with the Lincoln County Health Department and is following recommendations by the CDC, state and local health officials.

Indoor, in-room visitation is currently allowed at Pine Crest and began on March 29. Open visitation hours are 9am-6pm M-F and 9am-3pm on Weekends.

To control the volume of people in the building at one time, we encourage a 2-person visitor limit per resident during each visit, so large groups of visitors will not be permitted. The following infection prevention requirements will be in effect for all visitors:

  • Upon arrival, Covid-19 screening is required including questions about your Covid-19 vaccination status, signs of illness, exposure to Covid-19 in last 14 days and completing a temperature check.
  • Masks must be worn at all times and fully cover your nose and mouth. Masks will be provided.
  • Alcohol-based hand sanitizer will be required before proceeding into facility and will be provided.
  • Visitors will maintain 6-foot social distancing at all times.
  • Visitors are not allowed to access other parts of building other than designated visiting space or a resident’s room. You must immediately exit the facility after your visit. Bathroom use will be permitted.
  • Residents in semi-private rooms and those in rehab will need to call ahead so appropriate arrangement can be made when conducting these visits.

All visits will be indoors until warmer weather comes our way.

Since our last phone notification on March 19 there have been no new resident or employee Covid-19 cases reported and there are currently no active resident cases at Pine Crest. Our team continues close contact with the Lincoln County Health Department and is following recommendations by the CDC, state and local health officials.

We are happy to announce that recent federal and state guidelines on nursing home visitation restrictions have been updated due to ongoing vaccination efforts and a decrease in positivity rates in our community. Because of this, we are excited to share that indoor, in-room visitation will now be allowed at Pine Crest, beginning Monday, March 29. Open visitation hours will be 9am-6pm M-F and 9am-3pm on Weekends.

On another good note, Pine Crest is also bringing back beautician services for our residents beginning next week. North Central Health Care has been informed of one employee who has tested positive for Covid-19. The employee last worked on February 5 in Long Term Care. The employee was asymptomatic at work, wore the appropriate PPE at all times, and was not in the building during a period of possible transmissibility. We have no Covid-19 positive resident cases at this time.

We will continue with our diligent infection prevention practices including employee screening at multiple time per shift, wiping frequently touched surfaces and wearing masks and appropriate PPE to keep our residents, employees and community healthy.

Our team continues close contact with the Lincoln County Health Department and is following recommendations by the CDC, state and local health officials.

Outdoor visitation is closed at this time, virtual and compassionate care visits continue. Window visits are now available to be scheduled. Window visits are taking place in the main entrance vestibule. Window visits will run on Mondays, Tuesdays, Wednesdays and Fridays from 8 am-7 pm, Thursdays from 8am-4pm, and Saturdays and Sundays from 11 am-5:30 pm. Window visits are limited to 30 minutes. All window visits will be scheduled through our Life Enrichment department by calling 715-539-2549.

The windows will remain closed at all times and whiteboards will be provided as needed. Visitors are encouraged to bring a cell phone along as staff will assist residents with communicating and holding a phone as needed. Looking back we have not had a positive resident case since December 15 and have not had a positive employee case since January 20. We are very pleased to see this trend continue. We know that our diligent screening and infection prevention practices at all NCHC locations are working to prevent illness, but we will remain committed to these practices to keep our residents, employees and community healthy. Since our last phone notification on January 15, there been no new resident Covid-19 cases reported at Pine Crest. We have been informed that one employee did test positive for Covid-19 earlier this week through routine testing which was reported out on our website. The employee last worked on our Rehab unit on January 20, was not experiencing any symptoms while at work, however was in the building during a period of possible transmissibility. This employee is the only active positive employee currently reported at Pine Crest and currently there are no positive residents within the facility. We are very pleased to see this trend continuing across all our programs. We will continue with diligent screening and infection prevention practices at all NCHC locations, including routine employee Covid-19 testing twice per week.

North Central Health Care has been informed of one employee who has tested positive for Covid-19 through routine testing. The employee last worked on January 20 in Rehab. Upon notification of the positive result, the employee immediately stopped all resident care and left the building. The employee was asymptomatic at work and wore the appropriate PPE when providing residetn care. We have no new Covid-19 postive resident cases reported from routine testing.

Since our last phone notification on January 7, there been no new resident Covid-19 cases reported at Pine Crest and currently there are no positive residents within the facility. We have been informed that one employee has tested positive for Covid-19 earlier this week which was reported out below. The employee did not work in direct care, last worked on January 4 and was not in the building during a period of possible transmissibility.

Our team continues close contact with the Lincoln County Health Department and is following recommendations by the CDC, state and local health officials. Outdoor scheduled visitation is closed at this time, while virtual and compassionate care visits continue.

Earlier this week, partnered with Walgreens Pharmacy, Pine Crest held the first Covid-19 vaccine clinic scheduled for residents and staff. 67 residents received their first dose of the vaccine along with 68 staff. Additional clinics will be held in February and March to administer 1st and 2nd doses of the vaccine to residents and employees.

We have received positive feedback from residents and staff with how smoothly the clinic operated. We again want to thank you for your questions and for helping our residents get their consent forms completed. In total North Central Health Care has administered over 600 first dose Covid-19 vaccines to residents and staff in the last few weeks.

The employee last worked on January 4 in Food Services, does not provide direct care to residents and was not in the building during a period of possible transmissibility. We have no new Covid-19 postive resident cases reported from routine testing.

We welcome in 2021 with signs of hope and optimism for what our new year will bring. To start off with some good news, since our last phone notification on December 23 there have been no new resident or employee Covid-19 cases reported at Pine Crest and currently there are no positive residents within the facility.

We will continue diligent screening and routine testing of staff 2 times per week. Our team continues close contact with the Lincoln County Health Department and is following recommendations by the CDC, state and local health officials. Outdoor scheduled visitation is closed at this time, while virtual and compassionate care visits continue.

On a second bit of good news, Pine Crest staff have been preparing over the last several weeks and anxiously awaiting the Covid-19 vaccine arrival. We are pleased to share we have Pine Crest’s first Covid-19 vaccine clinic scheduled for residents and staff and on January 13. Nursing home residents who have completed the consent form will be first to receive the vaccine along with front line health care workers at Pine Crest.

Over the last week, Mount View Care Center in Wausau, also operated by NCHC, held our first vaccine clinics and successfully administered the first dose of the Covid-19 vaccine to 117 residents and over 300 front line staff. We have received positive feedback from residents and staff with how smoothly the clinic operated. Clinics are run in partnership with Walgreens Pharmacy. Since our last phone notification on December 18 there have been no new resident cases reported at Pine Crest and currently there are no positive Covid-19 residents within the facility. Pine Crest has had one staff member test positive for Covid-19 since our last report. The individual is a contracted employee and did provide care in several units in Pine Crest on December 19 including the 200, 600, 700, 800 and 900 units. The employee was in the building during a period of possible transmissibility.

Routine resident testing will continue weekly and staff testing will take place 2 times per week. Our team continues close contact with the Lincoln County Health Department and is following recommendations by the CDC, state and local health officials. Outdoor scheduled visitation is closed at this time. Virtual and compassionate care visits will continue.

As we await the arrival of the vaccine to our facility, we have created several resources for you and your family to review online including videos, links and frequently asked questions and answers. If you have not already returned the consent or declination form as a representative, please do so as soon as possible, so that we are prepared to administer vaccines when they arrive.

Virtual Visitation for the Holidays will be available on December 24 and 25. On Christmas Eve, North and Special Care Units will have virtual visitation from 6 am to 2:30 pm. South and Rehab units will be 8 am to 4:30 pm. On Christmas Day, North and Special Care Units will have virtual visitation from 11 am to 7 pm. South and Rehab units will be 8 am to 4:30 pm. These times and information will be available on our website to share with family.

Case Update | Monday, December 21, 2020

North Central Health Care has been informed of one employee who has tested positive for Covid-19. The direct care employee last worked on December 19 on the following units: 200, 600, 700, 800 and 900. The employee was in the building during a period of possible transmissibility. We have no new Covid-19 postive resident cases from routine testing. As of December 21, 2020 at end of day, Pine Crest will have no residents on the 400 unit who are active Covid-19 cases and the unit will be closed. We will continue with twice weekly testing of staff and once weekly testing of residents.

Since our last phone notification on December 11 there have been 2 new resident cases reported at Pine Crest. There are currently 2 residents total within the facility who are positive for Covid-19 with all residents located on the 400 unit.

In the last week, there has also been 1 new employee case reported. All other staff have recovered and been cleared to return to work using the CDC guidelines established. All case details can be found online on our website. While our Covid-19 case reports have drastically reduced in the last week, our commitment to preventing the spread of illness has not changed. Our staff are still wearing the appropriate PPE at all times, monitoring for symptoms, screening multiple times per shift and staying home when they are ill.

Routine resident testing will continue weekly and staff testing will take place 2 times per week. North Central Health Care has been informed of two residents who have tested positive for Covid-19 through routine testing. The residents are from the Special Care and 600 Unit and have been relocated to the 400 Unit. There is a total of 3 residents who are Covid-19 positive on the 400 Unit.


Case Update | Monday, December 14, 2020

North Central Health Care has been informed of one employee from Long-Term Care who has tested positive for Covid-19. The employee last worked on December 10 and developed symptoms while at work. Upon self reporting symptoms, the employee was immediately removed from patient care and left the building. The employee was in the building during a period of possible transmissibility.

There are currently 9 residents total within the facility who are positive for Covid-19 with all residents located on the 400 unit. In the last week, there has also been 7 new employees case reported. All other staff have recovered and been cleared to return to work using CDC guidelines established. Case Update | Thursday, December 10, 2020

North Central Health Care has been informed of 2 employees who have tested positive for Covid-19. One employee has not been in the building since November 26 and was not present during a period of possible ransmissibility. The second employee developed symptoms while working on Dec. 8 in a support role at the nurses station on South, and immediately stopped all patient care, self-reported and left the facility. There are no additional reports of positive residents at this time and currently there are 8 residetns within the facility who are Covid-19 postive.

Updated: 6:00 pm: North Central Health Care has been informed of 1 resident who have tested positive for Covid-19 through routine testing. The resident from the Special Care unit has been relocated to the 400 unit where currently all positive residents are located.

North Central Health Care has been informed of 2 residents who have tested positive for Covid-19 through routine testing. Both residents were on the Special Care unit and have been relocated to the 400 unit where currently all positive residents are located. Staffing is dedicated on this unit, which means employees will not be providing care in other units.


Case Update | Monday, December 7, 2020

North Central Health Care has been informed of 3 additional residents who have tested positive for Covid-19 through routine testing. Two of the residents were on the 600 unit and one was from the 700 unit. All residents have been relocated to the 400 unit where currently all positive residents are located. Staffing is dedicated on this unit, which means employees will not be providing care in other units. Additional routine resident tests results are still pending as of the time of this report.

Updated 6:00 pm: NCHC has also been informed of one additional employee who has tested positive for Covid-19. The employee works in Housekeeping and has not been in the building since November 25. This would mean they were not in building during period of possible transmissibility.

North Central Health Care has been informed of 3 additional employees who have tested positive for Covid-19 through routine testing. The employees were not symptomatic at work and wore a mask and the appropriate PPE at all times, however were in the building during a period of possible transmissibility. The employees worked in Special Care and the North units.

In the last week, there has also been 8 new employees case reported. Pine Crest currently has 8 staff total who tested positive for Covid-19 and are not working. All other staff have recovered and been cleared to return to work using CDC guidelines established.

All case details can be found below. Routine resident testing will continue weekly and staff testing will take place 2 times per week. Our team continues close contact with the Lincoln County Health Department and is following recommendations by the CDC, state and local health officials. Outdoor scheduled visitation is closed at this time. Virtual and compassionate care visits will continue.

Case Update | Thursday, December 3, 2020

North Central Health Care has been informed of 2 additional employees who have tested positive for Covid-19. One employee last worked on November 29 in Special Care as a hospitality aide. The employee was asymptomatic at work, wore a mask and the appropriate PPE at all times and was in the building during a period pf possible transmissibility. The second employee last worked on November 25 in North as a hospitality aide. This employee was asymptomatic at work, wore a mask and the appropriate PPE at all times, however was not in the building during a period pf possible transmissibility.

UPDATED: North Central Health Care has been informed of 1 additional employee who has tested positive for Covid-19 through routine testing. The employee last worked today in the 900 Unit and developed symptoms at work. The employee was immediately removed from patient care and left the building. The results from routine testing were received later on in afternoon after the employee had left. The employee was in the building during a period of possible transmissibility, however wore a mask and the appropriate PPE at all times.

North Central Health Care has been informed of 8 additional residents and 1 employee who have tested positive for Covid-19 as a result of routine testing. The residents were from the following units: 3 from 600 unit, 1 from 700 unit, 1 form 800 unit, 1 from 900 unit and 2 from Special Care unit. All residents have been relocated to the 400 Unit, where currently all positive residents are located.

The employee that tested positive as part of routine testing, last worked on December 1 in Food Services and does not provide direct patient care. The employee was asymptomatic at work, wore a mask and the appropriate PPE at all times, however was in the building during a period pf possible transmissibility.


Case Update | Monday, November 30, 2020

North Central Health Care has been informed of 1 additional resident and 3 employees who have tested positive for Covid-19 as a result of routine testing. The resident was from the 200 unit and has been relocated to the 400 Unit, where currently all positive residents are located. Two of the employees provided direct care and worked on several units prior to Pine Crest receiving the positive result notification, including the 100, 200, 700, 900, Special Care and Rehab Units. The employees were asymptomatic at work, wore masks and the appropriate PPE at all times, however were in the building during a period pf possible transmissibility. The third employee worked in laundry and housekeeping and does not provide direct care to residents.


Case Update | Friday, November 27, 2020

North Central Health Care has been informed of seven additional residents who have tested positive for Covid-19 as part of routine testing. The residents were from the 800 (4 residents) & 900 (21 resident) units and also Special Care (2 residents). All residents have been relocated to the 400 Unit, where currently all positive residents are located. One additional employee from Long Term Care also tested positive for Covid-19 through routine testing.

North Central Health Care has been informed of one additional resident in 800 Unit who has tested positive for Covid-19. The resident has been relocated to the 400 Unit, where currently all positive residents are located. Staffing in this area is dedicated to the care of these residents and will not be working in other parts of the building.
