NCHC Tomahawk Office Closure Notice

North Central Health Care would like to announce the closure of services at the Lincoln County Annex Building, known as the Tomahawk Office, effective December 11, 2020. NCHC Outpatient Services along with several other Lincoln County partner agencies used the office located at 213 W. Wisconsin Avenue in Tomahawk to provide care and services. Other businesses that were operated and not operated by NCHC in that office space are Wisconsin Home Energy Assistance Program (WHEAP), Social Services and Veteran’s Services.

NCHC continues its commitment to serving Lincoln County residents and is confident that we can meet the needs of consumers through the expansion of telehealth, non-emergency medical transportation for appointments and community based services. A letter was sent to all active clients of NCHC to notify them of this change and transition their care appropriately. 

NCHC will continue to monitor patient experience results and respond accordingly to best serve our clients and patients in northern Lincoln County. The last day that NCHC services within the Tomahawk Office will be provided is December 11, 2020.
