Report a Concernreporting a concern NCHC

If you suspect a violation of the Code of Conduct you can report anonymously anytime through our online Report a Concern form. Once submitted it is sent directly to the NCHC Compliance Officer. For immediate concerns outside of business hours you can contact the administrator on-call by dialing 715.848.4488. Other options for reporting a concern can be found in our Code of Conduct handbook. You can also find frequently asked questions that might help you avoid a potential violation.


NCHC is committed to ensuring that you do not face retaliation for reporting concerns about actions that may violate or be inconsistent with our Code of Conduct or the law.

Prohibited acts of retaliation include discharge, demotion, suspension, harassment, threats, or any other action that discriminates against an individual who submits a report.
