NCHC Employee Vaccination Program

(updated 9/30/2024)

Getting vaccinations, including the Influenza and Covid vaccinations, is an easy way to protect yourself, your family, your friends, your coworkers, and those we serve. The flu and Covid vaccinations are fast, affordable and safe, and can prevent severe illness and even death. The Centers for Disease Control and Prevention recommends that all people 6 months of age and older get vaccinated against the Influenza and Covid.

NCHC's Vaccination Policy requires ALL employees to participate in the vaccination process by either receiving vaccinations OR completing the required declination form.

ALL STAFF at ALL LOCATIONS will be offered both the Covid and Influenza vaccinations in Fall of 2024.

What You Need to Do to meet the NCHC Employee Vaccination Program Requirements

Employees have requirements for both Influenza and Covid vaccinations, as well as completing the annual TB Risk Assessment. Employees who do not complete these steps by a designated date will be removed from the schedule.

If you are a new hire, your manager or HR will deliver all required forms and instructions directly to you at work, to be completed by a designated date.

What Forms Do I Need to Complete to Meet My Requirements?

Each employee must complete the following forms and turn into Employee Health at a Vaccination Clinic or via Interoffice Mail to Renee Erickson in Employee Health:

  1. Influenza Vaccination Form OR Declination Form;
  2. Covid Vaccination Form OR Declination Form;

AND

  1. TB Risk Assessment Form

Fall 2024 Vaccination Clinics & Nurse Vaccinators

The Covid-19 and Influenza vaccines will be offered at NCHC to ALL NCHC Employees this year at ALL locations. Employees are not mandated to receive the Influenza or Covid vaccinations, however, you are required to participate in the Employee Vaccination Program, which requires each employee to complete the necessary forms for receiving or declining vaccinations.

Getting the Influenza or CovidVaccine At Work

We have a list of available Employee Open Vaccination Clinics. See graphic above the for dates and times. For those in Direct Care programs, a nurse vaccinator will be available to administer Influenza or Covid vaccines. Please contact your manager directly for more information.

Getting the Influenza or Covid Vaccine Outside of NCHC

Employees may choose to receive the Influenza and/or Covid vaccinations outside of NCHC at a primary care provider or community vaccinator, such as a local pharmacy. For employees who choose to receive vaccinations outside of NCHC, these will be done at your expense. Please remember to get a proof of vaccination form from your provider and turn in with your other forms to Employee Health by November 15, 2024.

Declining the Influenza and/or CovidVaccine

If you plan on declining the Influenza and/or Covid vaccine, declination forms are available at the above links. All forms must be turned in to Employee Health by November 15, 2024, or you will be removed from the schedule.

Vaccination Program FAQ’s

We know you may have questions. Check out these common questions and answers below.

Which vaccines are required for employees to receive?

  • All staff must participate in the Vaccination Program, meaning getting vaccinated or turning in the appropriate declination form.
  • All staff must get the influenza vaccine or turn in a declination form.
  • Only employees in MVCC, Pine Crest, Food Services, Housekeeping, and Transportation are required get the Covid vaccine and the Influenza vaccine or turn in the declination form. You may choose to decline one or both vaccines.

Can I decline one or both vaccines?

  • Yes. If you decline one or both, please complete the appropriate declination form and return to Employee Health with your other required forms by November 15. See forms required above.

If I decline the Influenza and/or Covid Vaccine, do I need to wear a mask for a period of time?

  • No. At this time, masks will only be required by all staff in areas designated due to outbreaks within the facility.

Can I get one or both vaccinations outside NCHC?

  • Yes. Should an employee get a vaccination outside NCHC, they will still need to complete the forms and return to Employee Health by November 15, 2024 with the forms provided to them and proof of vaccination.

How do I get forms to Employee Health?

  • Complete forms and place in a sealed interoffice envelope. Managers may choose to collect the envelopes to make sure their staff turn them in, or employees may return them in one of the following ways: at a vaccination clinic, interoffice mail, giving sealed envelope to a nurse vaccinator or dropping off at Employee Health or Human Resources.

Where can I sign up for an Employee Vaccination Clinic?

  • All vaccination clinics will be open to all staff. No appointment or sign-up needed. See the graphic above for all posted dates and times available.

When and Where are Employee Vaccination Clinics?

  • NCHC will hold Open Employee Vaccination Clinics onsite at all locations, as well as provide opportunities for direct care staff to have a nurse vaccinator administer your vaccines within your departments. Check out the graphic above for a list of dates and times.

How do I connect with a Nurse Vaccinator from my department?

  • Managers will be working with direct care staff designated as Nurse Vaccinators. Each direct care department will communicate to their team in the best way that works for their team. Contact your manager for more details.

I do not work in a direct care program, can I get my vaccination with a Nurse Vaccinator?

  • No. Due to volume and time available from our Nurse Vaccinators, these appointments are reserved for staff in direct care programs only. Please make an appointment with a local vaccinator or your primary care provider in the community.

What if I miss the Open Employee Vaccination Clinics?

  • We encourage staff to plan ahead and make it to an Employee Open Vaccination Clinic, but we understand that sometimes things happen that are out of your control, and you can't make your planned clinic time. If you miss the Open Employee Vaccination Clinics offered and still would like your flu or Covid vaccinations, you will be responsible to get the Covid vaccination outside of NCHC at your expense with a provider or community vaccinator of your choice. There is a chance that NCHC may have extra Covid vaccinations still available for a short time after the Employee Open Vaccinations Clinics end, however this is not guaranteed. Employees should not plan for this scenario. For those wanting the flu vaccine or to check on availability of the Covid vaccine, please contact Renee in Employee Health at 715.848.4396.

Need to Report A Concern?

Do you have a concern that you would like to bring forward? Do you suspect a violation of our policies or Code of Conduct? Please visit our online submission page: Report a Concern. Employees may submit concerns about compliance, ethics, client/patient/resident care, or any other concern you may have. These concerns can be submitted anonymously or you may leave you name for follow up.

Once submitted it is sent directly to the NCHC Compliance Officer. For immediate concerns outside of business hours you can contact the administrator on-call by dialing 715.848.4488. You should also report a concern to your direct supervisor or Department Director.

Recently change your network password and need help logging into UKG?

For network security, employees are required to change passwords at regular intervals. With our current systems and UKG, we are allowed to use something called Single Sign-On (SSO). SSO is a system that allows you to use one set of login credentials to access multiple websites or applications. Instead of remembering multiple usernames and passwords, you log in once with a central SSO service. This service verifies your credentials and generates a token that grants you access to other sites without additional logins. SSO simplifies the login process, saves time, and improves security by reducing the number of credentials to manage.

When you change your network password, all the applications that utilize SSO, such as UKG, will also have an updated password needed when you log in outside the SSO system (like on your phone or non-network computer).

1. VERY IMPORTANT: When you change your Network password, use the new password for the SSO sign in on your desktop or phone.

2. Mobile APP

  • After you have set up UKG via the desktop icon, you can then set up your Mobile App.
  • Install the UKG app
  • Facility code: nchc1
  • When the screen below displays, tap the blue bar – Use SSO
  • Enter your norcen.org email and your Network password.
